Current privacy laws in Ontario do not allow for the KidneyCare Outreach team to contact you directly without your patient’s consent. If you have not received it, please ask your patient directly.

The KidneyCare Outreach program is a new initiative developed by ICES Kidney, Dialysis and Transplant (KDT) Program from the ICES Western Site. KidneyCare Outreach is a pragmatic randomized clinical trial, using health administrative data from ICES, to identify individuals who may have unrecognized chronic kidney disease. ICES will be acting as the health information custodian to use and disclose personal health information for this research.
