You are encouraged to contact your family healthcare provider about your kidney health. You can stop taking part in the kidney health check at any time. If you stop, we will not collect any new information directly from you. ICES regularly receives health information from Ontario’s publicly funded health care system as part of its usual work. ICES uses this information to help improve health care across the province. Information about you that is already routinely provided to ICES will still be used in this research even if you withdraw. ICES takes many steps to protect people’s privacy. These privacy practices and procedures are reviewed and approved by the Information and Privacy Commissioner of Ontario. The information is coded, which means names and other identifying details are removed before it is analyzed. Results are reported only in a way that no individual can be identified.
